Coverage is NOT AVAILABLE to any individual who:

a) has been diagnosed with a terminal illness; or

b) has Acquired Immune Deficiency Syndrome (AIDS) or Human

Immunodeficiency Virus (HIV); or

c) has been prescribed or used home oxygen treatment in the last 12 months; or

d) has been diagnosed with or treated for congestive heartfailure; or

e) has had a major organ transplant (heart, kidney, liver, lung);or

f) has received kidney dialysis treatment in the last 12 months.

To be eligible for coverage you must, as of the effective date:

a) be at least 15 days old; and

b) be in good health at the time you purchase your policy andon the effective date, and know of no reason why you wouldattend any medical consultation during the period ofcoverage; and

c) not be insured or eligible for benefits under a CanadianGovernment health insurance plan. If you become eligible forand insured under the government health insurance plan ofthe province or territory in which you reside, the insurancewill apply only to those benefits not provided under theProvincial or territorial government health insurance plan.

Pre-existing conditions Age 0 to 79

Option 1: Covered only if stable during the 120 days prior to the effective date

Option 2: All pre-existing conditions will be excluded from coverage

Age 80 and over – All pre-existing conditions will be excluded from coverage

Pre-existing medical condition means a sickness, injury or medical condition, whether or not diagnosed by a physician:

i. for which you exhibited signs or symptoms; or

ii. for which you required or received medical consultation; and

iii. which existed prior to the effective date of your coverage

Stable means a pre-existing medical condition that:

a) did not require, or was not referred for any medical consultation;

b) did not require a change in type or dosage of medication.

Age 0 to 85

If this insurance is purchased any time after your arrival in Canada, then in respect of any sickness you will onlybe entitled to receive benefits for the cost of eligible medical expenses incurred after the first 48 hours from theeffective date of the policy.

Age 86 and over

If the insurance is purchased after your arrival date in Canada, then in respect of any sickness you will only beentitled to receive benefits for the cost of eligible medical expenses incurred after fifteen (15) days from theeffective date of this policy.

  • Drugs or Medications Up to $500 not exceeding a one-month supply
  • Transportation of Family Up to $3,000
  • Professional Services Physiotherapist, chiropractor, chiropodist, etc up to $500 per practitioner
  • Attendant up to $500
  • Transportation of Family/Friend up to $3,000
  • Follow-up visits up to $3,000
  • Accidental Dental up to $3,000
  • Dental Emergencies up to $500
  • Meals and Accommodation up to $1,500
  • Emergency Return Home up to $3,000

Under no condition will a refund be made if a claim has been incurred or paid, or is pending.

Partial refunds will be calculated on a pro-rated basis and are subject to a $25.00 administration fee and a minimum refund of$25.00.

A fee of $150 may apply if cancelling a policy issued for one year or more of consecutive coverage prior to the effective date.

Limits on Coverage

If you fail to notify SelectCare Worldwide, without reasonable cause, it will result in the reduction of your eligible benefit amounts payableby 20%.
